Stem cell treatment for the face and anti-aging.

Clinics sell three things under this name: a facial serum applied after microneedling, a fat graft called a stem cell facelift, and an anti-ageing IV drip. None is approved for cosmetic use anywhere, and the evidence for each is small and short.[1][2][3]

A stem cell facial and a stem cell facelift are different things

Stem cell facial

A serum of exosomes or the liquid stem cells were grown in, spread on the skin straight after microneedling or laser. It contains no living cells.[1][4]

Stem cell facelift

Fat taken by liposuction and injected back into the face, sometimes with extra cells from the fat added. The American Society of Plastic Surgeons says procedures sold as stem cell facelifts are often just fat injection, with no lasting anti-ageing effect.[5]

Anti-ageing IV drip

Donor cells infused into a vein, sold for energy and longevity. The only trials enrolled frail older adults and measured walking distance, not appearance or lifespan.[3] About IV stem cell therapy

What to expect

The procedure

A facial takes one clinic visit, with the serum applied after the needling. A fat graft adds a liposuction under local anaesthetic. Nobody in our review of patient accounts reported pain from a facial injection, time off work or an overnight stay.[6]

How many sessions

Trials of facials used three sessions three to four weeks apart.[7][8] Fat grafting is one operation in trials, though repeat operations are common.[9]

When it works, and how long it lasts

Clinics quote three to six months, and patients who gave a timescale agree.[6] Facial trials measured results at 8 to 12 weeks, and no trial in this area followed people beyond a year.[7][10] On average a facial fat graft keeps about 47% of the volume injected.[11]

Risks

Facials cause short-lived redness.[12] Fat injected into the face can block an artery: 61 published cases, including permanent blindness and six deaths.[13] Exosomes injected into the skin, rather than applied on top, have caused lumps with permanent scarring and skin death.[14][15]

Facials help a little; added cells do not improve the skin

Does it work? For a facial, a small improvement over a few weeks, most of which the needling may explain. For a facelift, extra cells keep more of the fat but do nothing extra for the skin.

Where it is allowed

United States. No cell product is approved for any cosmetic or anti-ageing use.[16] The FDA has warned a company selling exosomes with wrinkle claims that they are unapproved drugs.[17]

Malaysia. The regulator does not allow cell products to be sold for cosmetic, anti-ageing or rejuvenation use without clinical proof. Exosomes and secretomes fall outside the cell therapy rules altogether.[18]

Mexico. COFEPRIS has said stem cell treatments lack authorisation.[19]

Specialists. The two main US plastic surgery societies say stem cell marketing in aesthetic surgery is not supported by evidence, and that fat grafting should be called fat grafting.[2]

Is it better than PRP?

In the one head-to-head facial trial, both sides improved equally, and both had the same microneedling.[20] PRP vs stem cells

Common questions

Can an IV drip slow ageing?

No trial has tested it. The three randomised trials enrolled frail older adults and none measured lifespan, biological age or appearance.[3]

Questions to ask a clinic

  1. What exactly are the stem cells: plant extract, secretome, exosomes or my own fat?
  2. Is the product applied after needling, or injected into the skin?
  3. Which lab made it, and can I see the certificate for this batch?
  4. If it is a fat graft, how much fat usually survives?
  5. Who injects facial fat here, and how often do they do it?
  6. How many sessions, and what should I expect to see after each?
